The
background to the Joint Commission directive was a report entitled To Err is
Human, published by the National Academy of Sciences, Institute of Medicine
(IOM). The “To Err Is Human” exposed surrounded medical errors and its concern but
it is not blaming professional’s health care that does honest errors. Nevertheless,
to err is human.
